Research involving transgender and gender diverse people (TGD) increased in the last years, mostly concerning healthcare associated to this population. Few studies dedicated their analysis to the impact of parental support on transgender people, even though this is an important aspect in creating a safe environment on which these individuals can build their identity. In addition, the link between family support, TGD identity and homelessness is not completely established. Thus, due to the specificities of the family context of TGD individuals, (...) the aim of this study is to investigate the association between family support and TGD in different moments of the process of gender affirmation. In addition, this study also aims to explore the relationship between a lack of social support and low self-esteem, home abandonment, and dwelling in the street. The survey was designed based on the TransPULSE project and was made available in electronic format. The sample was constituted of 423 TGD residents in two Brazilian states. A Structural Equation Model analysis suggested that the impact of gender affirmation status on homelessness was mediated by parental support, through self-esteem, and the need to move from home. The association between the status of the gender affirmation procedures, family support and self-esteem was significant and indicated that the further TGD individuals advanced in gender affirmation, the more self-esteem and family support they would have. The association between family support and self-esteem indicated that family support was associated with higher self-esteem. Low family support was associated with the willingness to move from home due to one’s TGD status and there was also a significant correlation between low self-esteem and the willingness to move from home due to one’s TGD status. Finally, homelessness was associated with the willingness to move with a large effect size. Limitations include the sample that was constituted by individuals with Internet access and who had more contact with TGD communities. The findings indicate directions for interventions involving TGD people and their families, considering the parental relationship as a critical variable to improve TGD quality of life in the process of gender affirmation. The article aims at challenging the very idea of bioethics. Starting with a thought experiment which puts into perspective relations between law and ethics, a reflexion on founding moments of bioethics as a distinctive discipline will be proposed in order to contest some of its assumptions. Less traditional ways of seeing the field will be subsequently analyzed, yet the conclusion of this exploration will remain critical: we should rather think of an alternative way of seeing moral expertise in the biomedical (...) sphere, founded on plural knowledge of diverse agents who leave their moral presuppositions aside as much as this is possible. The French Convention on Bioethics (États généraux de la bioéthique, 2009) were a perfect spot to observe new methods of organizing the participants of public debates into a hierarchy. Indeed, a privileged place was given to the citizen, and quite often the expert and the politician were devoid of their usual roles. This article will focus on the analysis of the very role given to the expert and to the expertise, both in the definition of the general framework of the (...) series of meetings and in public debates that took place in (among others) Marseille and Paris. This analysis makes obvious the idea that the major result of those Conventions does not consist in putting forward some original thoughts on biomedicine and its problems, but rather in promoting a new model of governance. In consequence, and if I am right, this promotion is being made precisely by diminishing the role of the expert (the intellectual). (shrink)

8 March, now known as International Women’s Day, is a day for feminist claims where demonstrations are organized in over 150 countries, with the participation of millions of women all around the world. These demonstrations can be viewed as collective rituals and thus focus attention on the processes that facilitate different psychosocial effects. This work aims to explore the mechanisms involved in participation in the demonstrations of 8 March 2020, collective and ritualized feminist actions, and their correlates associated with personal (...) well-being and collective well-being, collective efficacy and collective growth, and behavioral intention to support the fight for women’s rights. To this end, a cross-cultural study was conducted with the participation of 2,854 people from countries in Latin America and Europe, with a retrospective correlational cross-sectional design and a convenience sample. Participants were divided between demonstration participants and non-demonstrators or followers who monitored participants through the media and social networks. Compared with non-demonstrators and with males, female and non-binary gender respondents had greater scores in mechanisms and criterion variables. Further random-effects model meta-analyses revealed that the perceived emotional synchrony was consistently associated with more proximal mechanisms, as well as with criterion variables. Finally, sequential moderation analyses showed that proposed mechanisms successfully mediated the effects of participation on every criterion variable. These results indicate that participation in 8M marches and demonstrations can be analyzed through the literature on collective rituals. As such, collective participation implies positive outcomes both individually and collectively, which are further reinforced through key psychological mechanisms, in line with a Durkheimian approach to collective rituals. The precarious rights of senior citizens, especially those who are highly educated and who are expected to counsel and guide the younger generations, has stimulated the creation internationally of advocacy associations and opinion leader groups. The strength of these groups, however, varies from country to country. In some countries, they are supported and are the focus of intense interest; in others, they are practically ignored. For this is reason we believe that the creation of a network of all these associations (...) is essential. The proposed network would act as a support for the already-existing policies of the United Nations’ High Commission for Human Rights, of independent experts, and of the Global Alliance for the Rights of Older People. All three have long ago recommended the creation of a recognized instrument for uniting presently scattered efforts. The proposed network, therefore, will seek to promote the international exchange of relevant expertise, and it will reinforce the commitments and actions that single countries are currently taking to meet these objectives. For example, informative public events can be organised to promote particular support initiatives and to provide an opportunity for new members of the network to be presented. The network will promote health for senior citizens, disease prevention, senior mobility, safe free time for seniors, alimentary education, protection against new risks and dangers, as well as equity in the services necessary for seniors to adopt new information and communication technologies. In the case of retired academic members, the network will promote equality with respect to continuing use of digital technologies (particularly email), continuing access to research libraries, and the guaranteed ability for seniors to fund their own research programs and to deliver free seminars. Si la praxéologie mérite aujourd’hui notre attention, c’est parce qu’elle permet de remonter aux origines des « théories de l’action », des sociologies de l’action ou des philosophies de l’action qui ont marqué le xxe siècle. Que veut dire, au fond, étudier l’action, en proposer ensuite une théorie, à visée soit descriptive soit normative? Dans ce qui suit, on va étudier la question en compagnie de Tadeusz Kotarbiński (1886-1981), philosophe analytique, et de Harold Garfinkel (1917-2011), sociologue hétérodoxe. Ce texte défend (...) l’idée selon laquelle parmi les conséquences les plus marquantes du travail de Garfinkel se trouve la démystification de la « théorie de l’action », et que certaines intuitions similaires sont déjà très présentes chez Kotarbiński (dont les travaux sur l’action constituent, selon Garfinkel, un « échec extrêmement important »). La praxéologie chez les deux auteurs n’est pas une discipline, mais une façon de voir la pratique étudiée comme un tout, à l’instar des recherches menées par les théoriciens de la Gestalt. La dimension athéorique de l’étude de l’action humaine proposée ici permet d’espérer qu’un jour, les phénomènes qui nous entourent vont pouvoir être étudiés pour ce qu’ils sont, et non pas pour ce qu’une théorie élégante permet d’en dire. Mastroianni (bio), Ruth Faden (bio), and Daniel Federman (bio)In recent years, claims have been made by segments of the research community and by women's health advocacy groups that clinical research practices and policies have not benefitted women's health to the same extent as men's health. Central to these claims has been an assertion that women have been inadequately (...) represented as subjects of clinical studies and that as a result neither health conditions unique to women—e.g., menopause—nor women's manifestations of health problems affecting both sexes—e.g., heart disease—have been investigated sufficiently.The scientific community, including federal agencies that sponsor and regulate clinical studies, is increasingly responsive to these claims and is taking steps to raise the level of women's participation in clinical studies. Controversy and concern have surrounded these actions, however. Two of the claims that have been made are: (1) that women are more difficult to study than men because of their cyclical hormonal changes; and (2) that conducting gender-specific subgroup analyses would increase the size of study populations, raise the cost of studies, and thereby reduce the number of studies that could be performed with the limited resources available. In addition, controversy over the inclusion of women of childbearing potential and pregnant women has been particularly salient. Concerns have been expressed about avoiding potential harm to existing or potential fetuses and about the possible legal and financial ramifications of such harm. A further concern involves the perceived difficulties in enrolling women in studies and retaining them for the duration of the studies.Against this backdrop, the Office of Research on Women's Health at the National Institutes of Health (NIH) asked the Institute of Medicine (IOM) in October 1992 to establish the Committee on the Ethical and Legal Issues Relating to the Inclusion of Women in Clinical Studies. The Committee's charge was to: (1) consider the ethical and legal implications of including women, particularly pregnant women and women of childbearing potential, in clinical studies; (2) examine known instances of litigation regarding injuries to research subjects [End Page 55] and describe existing legal liabilities and protections; and (3) provide practical advice on these issues for consideration by NIH, institutional review boards (IRBs), and clinical investigators.The 16 Committee members came from diverse backgrounds: bioethics, law, epidemiology and biostatistics, public health policy, obstetrics and gynecology, clinical research, pharmaceutical development, social and behavioral sciences, and clinical evaluative sciences.1 Chaired by two of the authors of this article, Ruth Faden and Daniel Federman, and coordinated by the third, IOM Study Director Anna Mastroianni, the Committee met five times over a fourteen-month period, convened a one-and-one-half day invitational workshop, and commissioned several background papers. The Committee's deliberations were complicated by the announcement of new federal policies late in its term. Specifically, the Food and Drug Administration (FDA) issued guidelines (FDA 1993) to replace its 1977 guidelines, which prohibited the inclusion of women of childbearing potential in early phases of most clinical drug trials. In addition, Congress passed the NIH Revitalization Act of 1993 (P.L. 103-43), which contains provisions mandating the inclusion of women and racial and ethnic minorities in NIH-sponsored clinical research. In February 1994, the IOM Committee publicly issued its final report and recommendations, Women and Health Research: Ethical and Legal Issues of Including Women in Clinical Studies, publishing the workshop presentations and commissioned papers in a separate volume.Deliberations of the CommitteeA number of issues arose at the outset as the Committee considered its charge. A few members initially believed that verification of the underrepresentation of women relative to men in clinical studies as a whole was a prerequisite to providing policy recommendations. The coronavirus disease-2019 (COVID-19) has caused shortages of life-sustaining medical resources, and future waves of the virus may cause further scarcity. The Yale New Haven Health System developed a triage protocol to allocate scarce medical resources during the COVID-19 pandemic, with the primary goal of saving the most lives possible, and a secondary goal of making triage assessments and decisions consistent, transparent, and fair. We outline the process of developing the protocol, summarize the protocol, and discuss the major ethical challenges (...) encountered, along with our answers to the challenges. These challenges include (1) the role of age and chronic comorbidities; (2) evaluating children and pregnant patients; (3) racial, ethnic, and socioeconomic disparities in health; (4) prioritization of healthcare workers; and (5) balancing clinical judgment versus protocolized assessments.We conclude with a review of the limitations of our protocol and the lessons learned. We hope that a robust public discussion of such protocols and the ethical challenges they raise will result in the fairest possible processes, less need for triage, and more lives saved during future waves of the pandemic and similar public health emergencies. BackgroundStructured, systematic methods to formulate consensus recommendations, such as the Delphi process or nominal group technique, among others, provide the opportunity to harness the knowledge of experts to support clinical decision making in areas of uncertainty. They are widely used in biomedical research, in particular where disease characteristics or resource limitations mean that high-quality evidence generation is difficult. However, poor reporting of methods used to reach a consensus – for example, not clearly explaining the definition of consensus, or not stating (...) how consensus group panellists were selected – can potentially undermine confidence in this type of research and hinder reproducibility. Our objective is therefore to systematically develop a reporting guideline to help the biomedical research and clinical practice community describe the methods or techniques used to reach consensus in a complete, transparent, and consistent manner.MethodsThe ACCORD project will take place in five stages and follow the EQUATOR Network guidance for the development of reporting guidelines. In Stage 1, a multidisciplinary Steering Committee has been established to lead and coordinate the guideline development process. In Stage 2, a systematic literature review will identify evidence on the quality of the reporting of consensus methodology, to obtain potential items for a reporting checklist. In Stage 3, Delphi methodology will be used to reach consensus regarding the checklist items, first among the Steering Committee, and then among a broader Delphi panel comprising participants with a range of expertise, including patient representatives. In Stage 4, the reporting guideline will be finalised in a consensus meeting, along with the production of an Explanation and Elaboration document. In Stage 5, we plan to publish the reporting guideline and E&E document in open-access journals, supported by presentations at appropriate events. Dissemination of the reporting guideline, including a website linked to social media channels, is crucial for the document to be implemented in practice.DiscussionThe ACCORD reporting guideline will provide a set of minimum items that should be reported about methods used to achieve consensus, including approaches ranging from simple unstructured opinion gatherings to highly structured processes. RationaleSurrogates of critically ill patients often have inaccurate expectations about prognosis. Yet there is little research on how intensive care unit clinicians should discuss prognosis, and existing expert opinion-based recommendations give only general guidance that has not been validated with surrogate decision makers.ObjectiveTo determine the perspectives of key stakeholders regarding how prognostic information should be conveyed in critical illness.MethodsThis was a multicenter study at three academic medical centers in California, Pennsylvania, and Washington. One hundred eighteen key stakeholders completed in-depth semistructured (...) interviews. Participants included 47 surrogates of adult patients with acute respiratory distress syndrome; 45 clinicians working in study ICUs, including physicians, nurses, social workers, and spiritual care providers; and 26 experts in health communication, decision science, ethics, family-centered care, geriatrics, healthcare disparities, palliative care, psychology, psychiatry, and critical care.Measurements and main resultsThere was broad support among surrogates for existing expert recommendations, including truthful prognostic disclosure, emotional support, tailoring the disclosure strategy to each family's needs, and checking for understanding. In addition, stakeholders offered suggestions that add specificity to existing recommendations, including: In addition to conveying prognostic estimates, clinicians should help families "see the prognosis for themselves" by showing families radiographic images and explaining the clinical significance of physical manifestations of severe disease at the bedside. Many physicians did not support using numeric estimates to convey prognosis to families, whereas many surrogates, clinicians from other disciplines, and experts believed numbers could be helpful. Clinicians should conceptualize prognostic communication as an iterative process that begins with a preliminary mention of the possibility of death early in the ICU stay and becomes more detailed as the clinical situation develops. Although prognostic information should be initially disclosed by physicians, other members of the multidisciplinary team-nurses, social workers, and spiritual care providers-should be given explicit role responsibilities to reinforce physicians' prognostications and help families process a poor prognosis emotionally.ConclusionsFamily members, clinicians, and experts identified specific communication behaviors that clinicians should use to discuss prognosis in the critical care setting. These findings extend existing opinion-based recommendations and should guide interventions to improve communication about prognosis in ICUs. (shrink)

A mi-chemin entre Vienne et Cambridge, c'est a Lvov, et puis partiellement a Varsovie, que Jan Lukasiewicz, Stanislaw Lesniewski, Alfred Tarski, Kazimierz Ajdukiewicz, Tadeusz Kotarbinski et bien d'autres encore, repenserent dans un esprit d'analyse les questions fondamentales de la philosophie du (...) langage, de la logique, de la philosophie des sciences et des mathematiques. Plus qu'une simple traduction, ce livre est une version revisee de la monographie desormais classique que Jan Wolenski, connu pour ses travaux non seulement en histoire de la philosophie analytique, mais aussi en epistemologie et en theorie de la verite, a consacree a l'Ecole de Lvov-Varsovie. Come pensavano il tempo i Greci di eta arcaica, prima che Platone ed Aristotele ne formulassero una definizione? Nei miti, il tempo della natura, ciclico e rassicurante, non era messo a confronto con quello umano, imprevedibile anche se inesorabilmente determinato. Il ritmo scandito dal sole non era sovrapponibile a quelle della vita umana, l'avvicendarsi delle stagioni non misurava la durata della stirpe. Solo in eta sapienziale (VII-V secolo a.C.) si comincio a pensare il tempo in modo onnicomprensivo e si avverti (...) l'esigenza di misurarne il flusso. In quell'epoca, Eraclito tratteggio una concezione del tempo assolutamente innovativa, capace di ribaltare le visione del mondo trasmessa dai miti. Penso al tempo delle stagioni in analogia con quello interiore e vide nell'inesauribile creativita dell'anima umana lo specchio dell'infinita potenzialita della natura. La concezione offerta da Eraclito, avversata dai filosofi di eta classica e misconosciuta dalla scienza, trasmette ancora oggi, a chi legge con attenzione i frammenti rimasti della sua opera, un fiducioso messaggio di speranza nella capacita umana di vivere creativamente il tempo. Hasard et création chez Merleau-Ponty -/- À plusieurs reprises, Merleau-Ponty tisse une correspondance entre art et histoire, entre pratique artistique et action politique : plus précisément il nous invite à former le concept d’histoire sur l’exemple de l’art. À première vue, un tel rapprochement pourrait paraître abstrait, sinon provocateur, l’art étant souvent conçu comme un domaine qui semble avoir peu à faire avec l’espace de l’action. Mais, nous pouvons aujourd’hui comprendre davantage (...) l’intérêt de l’intuition merleau-pontienne, nous, qui faisons l’expérience de la connexion étroite entre l’univers visuel et le milieu politique ou historique, comme des dimensions devenues désormais inséparables. Au fur et à mesure qu’une conception positiviste de l’histoire ou de la politique entendue comme progrès, ou du moins comme projet, échoue sous nos yeux interdits, la question de l’historicité se fait de plus en plus urgente : Merleau-Ponty nous invite à penser l’histoire comme le « lieu de nos interrogations et de nos étonnements », le lieu d’une réponse, voire d’une responsabilité que nos organismes technologiques exigent. C’est à partir de l’expérience de la création artistique et de son rapport constitutif au hasard qu’une pensée de l’historicité peut s’élaborer, là où l’esthétique merleau-pontienne rejoint le mystère d’un sens historique et esquisse implicitement une éthique de la contingence, en ce qu’elle nous appelle à un exercice de la liberté. -/- Abstract -/- Anna Caterina Dalmasso The Artist and the Adversity. Chance and Creation in Merleau-Ponty -/- Throughout his works, Merleau-Ponty has developed the analogy between art and history, between art practice and political action: more precisely he invites us to think about the concept of history on the example of art. At first sight, such an equivalence could seem abstract, if not provocative, insofar as art is still sometimes regarded as a field having little to share with action. But today, experiencing the close connection between the visual and the political environment, we can understand, perhaps better than his contemporaries, what is at stake in Merleau-Ponty’s insight for a new comprehension of history. Whereas a positivist conception of history and politics understood as progress or project seems to be failing before our astonished eyes, the question of historicity becomes more and more urgent: Merleau-Ponty prompts us to think of history as a mysterious junction between facticity and intention, as the beginning of our wonder, as the place of the responsibility that our technological organisms demand. The experience of creation is the figure through which Merleau-Ponty tries to conceive contingency, which is the kernel of the Geneva conference, later called The Man and the Adversity. So, drawing on Merleau-Ponty’s constant reference to the operation of artistic creation, especially on the reference to El Greco, I aim at examining how a new conception of history can spring by a comprehension of the experience of creation. I will argue that, from the analysis of the relationship between the artist and the sensible, between the artist and her body, Merleau-Ponty comes to outline what I would call an ethics of contingency, insofar as it calls us to the exercise of our freedom. O. Zuretti, il quale ne curò nel 1930 l'editio princeps nel VII volume del Catalogue des manuscrits alchimistes grecs, è «un traité pratique, complet et systematique, rédigé à l'époque médiévale par un Grec d'Italie du Sud. Celui-ci se base sur de nombreuses sources latines et arabo-latines, sans (...) renier pour autant la tradition grecque, et il écrit dans une langue déroutante … respectée au maximum» (p. VII). Come scrive l'Editrice nell'Avant-propos, questa sintesi delle conoscenze alchemiche mediterranee degli inizi del 1300 è il più lungo dei testi alchemici greci e presenta diversi motivi di interesse: • è il testimone del sincretismo culturale che fioriva nel regno di Napoli in epoca angioina; • è un documento interessante per lo studio della lingua greca medievale della Calabria; • permette di affrontare problemi filologici legati al fenomeno della traduzione. C’est précisément une telle méthode, que Guillaume Carron reprend de Merleau-Ponty dans son ouvrage La désillusion créatrice. The article is devoted to the analysis of the key directions of the study of the possible influence of the specifics of Chinese language culture on the content and nature of intellectual discourse, which is recognized as philosophical. Logic and ontology are the key areas of analysis of the possible influence of linguistic determinants on the intellectual discourse of China. Three main topics that attract the attention of researchers are the (...) structure of judgment, the semantics of lexical units, and the specificity of the pragmatics of the Chinese language. M e t h o d s. The author applied general scientific methods of analysis, synthesis, comparison and description of the works of researchers of Chinese philosophy, which justify and argue the fruitfulness of studying the peculiarities of Chinese language to explain the reasons of the fundamental difference between the problems actualized in the history of Chinese philosophy and those that formed the basis of the development of the history of philosophy in the Western world. R e s u l t s. It is generally accepted that for the worldview of representatives of Chinese culture, the world is a structural whole, consisting of elements that continuously interact in the dynamics of changes and transformations, we find a reflection of this feature in the semantics of the corresponding symbolic system. Polysemy in the Chinese language mostly reveals a complex and multi-layered "web" of associative and correlative connections between meanings, which in fact cannot be argued outside the context of the relevant cultural tradition indicating common features or similarities of the signified. The doubt about the typicality of understanding the denotations as fixed and atomized for the Chinese language culture is reasonable, even in those cases when it comes to the designation of empirically accessible objectivity. The main specific features of polysemy in the Chinese language are the complete heterogeneity of what is signified, and the lack of transparent grounds for identifying the genus-species subordination of the scope of general concepts. This can also serve as an explanation for the fact that we do not find a completely obvious and nondisputable analogue of the problem of universals of the Western model in the history of Chinese philosophy. C o n c l u s i o n s. Based on the peculiarities of the Chinese language culture, we can recognize as a reasonable a statement that the seemingly natural understanding of the essence – the subject, as something that remains the same (at least for a certain time), but it undergoes some changes through its manifestations – a predicate (properties and actions). It is not as necessary and axiomatic for Chinese thinkers as, for example, for ancient Greek philosophers. In addition, we can recognize the fact that the verb "to exist" does not fulfill the role of a necessary copula in judgments in the Chinese language, as a probable reason for the lack of fundamental problematization of the concept of "being" in the Chinese history of philosophy. In general, the context of the use of a certain word-sign in the Chinese language plays a determining role. The actualization of one or another meaning in an expression is formed not only by its integration into a certain text, but also by its purpose in accordance with the specific practice of application. Therefore, this can serve as an explanation for the absence of an original and axiomatic interpretation of truth as absolute, that is, unchanging and non-relative in the history of Chinese philosophy. Le but de cet article est d'apprécier l'interdépendance qui, en matière d'analyse linguistique comme ailleurs solidarise l'approche scientifique et l'objet qu'elle s'est donné.En la circonstance, il s'agira des définitions, des attentes de rendement, bref d'un ensemble de présupposés sous-jacents à l'établissement d'un corpus. L'analyse est ici conditionnée par la nature et les dimensions du terrain couvert, par les connaissances antérieures et les théories qui le préstructurent, mais il ne faut pas se leurrer, nature et dimensions du domaine n'ont pas de (...) légitimité a priori : toute pratique de coupes dans le continuum du réel requiert une justification. Même si certains découpages (périodisations, classifications) semblent naturels, l'évidence n'est que le fruit de nos habitudes culturelles, voire de conventions académiques. Bien des cloisons sont déplaçables, et la délimitation du champ est alors affaire de point de vue. C'est ce qu'on montrera en examinant deux approches connexes, l'analyse du discours vs la linguistique textuelle. Ces disciplines, quoique familières, n'échappent pas à une prévisible confusion, car dans les faits texte et discours ne sont pas séparables : ce n'est qu'à travers l'analyse des textes, objets empiriques, qu'un type de discours est connaissable. A rebours, la reconnaissance du discours manifesté est indispensable à l'interprétation de tel ou tel texte occurrent. Une troisième pratique, qui relie les étapes, trouve ici sa place : la stylistique aujourd'hui explicitement ancrée dans les « sciences du discours », et précisément caractérisée par sa gestion d'un double point de vue sur le corpus.
